For your convenience, we accept payments through Visa, MasterCard, Discover, American Express, cash and checks. We also participate with Care Credit, the health spending credit card which allows qualified patients twelve months to pay without interest.
We deliver the finest care at the most reasonable cost to our patients. Payment is due at the time service is rendered unless other arrangements have been made in advance. If you provide insurance coverage information, we will attempt to verify your benefits so that we can determine an ESTIMATE of your financial responsibility. We will collect a surgery deposit based on the information provided by your carrier. However, we cannot guarantee that you will not have a balance due after the claim has been paid.
Please remember you are fully responsible for all fees charged by this office regardless of your insurance coverage.
We will send you a monthly statement. Most insurance companies will respond within four to six weeks. Please call our office if your statement does not reflect your insurance payment within that time frame. Any remaining balance after your insurance has paid is your responsibility. Your prompt remittance is appreciated.

At Drs. Koslin, Kahn & Denson P.C. we make every effort to provide you with the finest care and the most convenient financial options. To accomplish this we work hand-in-hand with you to maximize your insurance reimbursement for covered procedures.
We accept most insurance plans but we encourage you to call your insurance company if you have any questions or concerns about your network or benefits. Any treatment costs not covered by insurance will be the responsibility of the patient.
If your plan requires a co-pay for medical visits, please be prepared to pay that on the day of the appointment.
We must make a copy of your insurance cards at your initial visit and ask that you inform our staff if your insurance changes at any time.
Prior to surgical procedures, our staff will attempt to verify your insurance benefits and determine your costs. However, we cannot guarantee payment by insurance companies. We will not have a final bill until after the claim is processed.
